For clinicians:

The DBT Diary Card is a digitized version of the diary card used in evidence-based DBT programs. This tool is a daily tracker of mood, target behaviors and the use coping skills to manage different emotions, urges, and behaviors. Our Dialectical Behavior Therapy applet will enable therapists to create a digital DBT diary card with a patient to include specific treatment targets for that individual. It will also use notifications to remind patients to complete the diary card on a daily basis. The patient’s device will automatically update with the new targets and schedules when changes are made by their therapist during treatment, allowing the user to progress in the therapy without having to change the way the data is retrieved. Both patients and therapists will have access to the diary card data to guide treatment planning and sessions.

For users:

Once you have registered for and installed the MindLogger app (see the User Guide) and accepted an email invitation to use the DBT Diary Card, you are ready to track your mood, urges, and specific behaviors, and to practice skills to manage these emotions, urges, and behaviors. You will receive daily notification reminders on your phone to complete your diary card. Please see the images below for demonstrations of how to fill out your diary card.
